There is a page 3 of the survey, the page past the demographic page, and it is the most important page. Only about half of the respondents accessed this page. You can not get to page 3 until you have finished page 2. If you click next when you are not done with page 2, then you will be returned to the top of page 2 and not forward to page 3. Solution - finish the demographic information and then NEXT will take you to page 3.
If you forgot to do page 3, you can return and complete the survey again. Thank you for you patience and help with this research.
The first survey that was completed properly was done on June 1, 2008. Our goal is 500 responses.
